What is Evidence of Insurability (EOI)?
EOI is an application process where you provide information on the condition of your health or your dependent's health to be considered for certain types of insurance coverage.
Moving forward, if you want to receive consideration for new or increased supplemental/voluntary life coverage for yourself or your dependent requiring an EOI process, you will need to follow the below instructions.

Options to submit a completed EOI form: Mail to: Dearborn Life Insurance Company Attn: Medical Underwriting Department P.O. Box 7072 or Fax to Dearborn: (855) 691-7157 Once Dearborn receives your completed EOI form, you will receive additional communication directly from Dearborn regarding additional requirements and/or information regarding the disposition of your EOI.
