New Evidence of Insurability process for life insurance

Previously, employees submitted Evidence of Insurability forms to HR Benefits to send to the insurance carrier for processing. To expedite and streamline the process, employees are now required to submit EOI forms directly to the insurance carrier.

Announcing a change to the Evidence of Insurability process for Supplemental/Voluntary Life Insurance enrollments and changes

What is Evidence of Insurability (EOI)?

 EOI is an application process where you provide information on the condition of your health or your dependent's health to be considered for certain types of insurance coverage.  

Moving forward, if you want to receive consideration for new or increased supplemental/voluntary life coverage for yourself or your dependent requiring an EOI process, you will need to follow the below instructions.

 

 

New STEPS to process EOI Forms in ESS is easy as 1,2,3

If you are prompted to complete the EOI process to during the life insurance enrollment process, you will need to:

  1. Download an EOI form.
  2. Completely answer all questions on the EOI form.
  3. Submit the completed and signed EOI form. Once you have fully completed and signed the EOI form, please follow the instructions on the top of the form and submit the completed form directly to Dearborn Life Insurance Company (Dearborn) as outlined below:

Once Dearborn receives your completed EOI form, you will receive additional communication directly from Dearborn regarding additional requirements and/or information regarding the disposition of your EOI.
