If you have elected health insurance under the COH Cigna Limited Plan and have a permanent address outside of the network area, you must act.

HR Benefits mailed letters to individuals on the City of Houston’s Cigna Limited HMO plan who have a permanent address outside the network area. If you receive this correspondence, that means you must change from the HMO to one of our two PPO plans.  

Failure to change from the HMO or relocate within the network area will result in a disruption to your health insurance benefits, since the Cigna Limited Plan is an HMO Plan designed to operate within a specified network.
